Our Process
We are here to help you through every step.
- First of all you need to attend an information seminar. To register you can call our Seminar Registration line at 866-338-7546(SLIM) or sign up here on line.
- Sign up for a consult with one of the surgeons at the end of the seminar.
- Contact your insurance company and check your benefits for weight loss surgery.
- Have your primary care physician complete the Primary Care Clearance form in your seminar packet.
- Make an appointment with the psychologist. (List will be provided at the seminar.)
- Attend a Support group meeting. You can call 937-208-4968 and select option #2 for information about upcoming meetings or click on Support group for the dates and times.
